November 2020 Newsletter

President's Message - Jim Ratcliff, DPM

Greetings to you from the Bay Area. Well, this Thanksgiving will be like none other. Most of us will have fewer family members or a “Zoom” turkey day.  We can still enjoy this great holiday with good food and football.

If your college-aged students come home for an extended break, try doing something different. Play scrabble, make cookies, bake bread, or just hang out. Exercise is a great way to be together outside and is a safe and healthy activity.

The pandemic has made many of us have “cabin fever.” Go for a long walk, run, or bike ride. Make sure you take your Vitamin D with food; it can definitely help your immune system. The vaccines look promising, so if we can hunker down for a few more months, hopefully our lives will return to normal soon.

I am reading Academy Award winning actor Matthew McConaughey’s new book, Greenlights. It is not Shakespeare, but he has a lighthearted way of describing his life. I needed something “light,” not too heavy and it has not disappointed.

As podiatrists, our job – helping folks stay on their feet – seems more important than ever during the pandemic as staying active is so important for our health, body, and soul.

I hope all of you have a great Thanksgiving.

Cheers,
Jimmy

AAPSM Past President Discusses Pandemic Problematic for Feet

The coronavirus crisis is beginning to impact people’s feet, according to AAPSM Past President Howard Osterman, DPM.  Dr. Osterman offers advice for what might help. “We’re actually seeing a huge array of foot issues that have come from people who are now homebound because of the pandemic.”

Dr. Howard Osterman

In addition to being an AAPSM Fellow and Past President, Dr. Osterman is the team podiatrist for the Washington Wizards and Washington Mystics and consults with Georgetown University sports teams. “We’re seeing a lot of heel pain; we’re seeing a lot of arch pain.”

AAPSM Past President Speaks at the
PFA Virtual Symposium

Dr. Paul Langer, DPM

AAPSM Fellow and Past President Paul Langer, DPM joined more than a dozen national experts for the first-ever virtual delivery of the Annual Pedorthic Footcare Association (PFA) Symposium. The virtual event was the PFA’s pandemic response to its annual traditional in-person clinical and technical educational conference for professionals specializing in the management and treatment of conditions of the foot, ankle, and lower extremities requiring fitting, fabricating, and adjustment of devices. Dr. Langer discussed how new scanning and 3D printing technology will become increasingly accessible to clinicians, retailers, and consumers. This access will allow for better precision in fitting shoes and the ability to produce customized footwear on demand on-site. The health benefits of comfort and the ability to address pathology were also discussed. “And comfort,” he explained, “is the opposite end of the pain spectrum. Comfort is very important and can even affect self-esteem.”

AAPSM Fellow Interviews Olympian on
Staying Healthy During the Pandemic

AAPSM Fellow Ben Pearl, DPM interviewed Past AAPSM Golden Foot Award recipient Gary Hall Jr. recently for his biweekly Fit Foot U program. The segment included Hall's commentary on overcoming the challenges of diabetes in sports, doping, harassment, and keeping physical and mental health during the COVID-19 pandemic.

Gary Hall, Jr. and Dr. Aaron Pearl

Hall noted that he has come across two different types of people regarding fitness during the pandemic. “There are those that say I just finished Netflix or HBO and those that tell him I'm in the best shape of my life,” says Hall. He encouraged a routine for exercise with small goals that are within reach. Dr. Pearl created the format to have a forum for athletes and doctors to discuss issues related to health, fitness, and sports medicine.

AAPSM Past President Participates Once
Again at Penn Relays

AAPSM Past President and Fellow Gary Gordon, DPM – who recently announced his retirement, continues to volunteer at the annual Penn Relays. where he has made many contributions throughout the years.

Special Olympics Fit Feet Update

Due to COVID, Special Olympics Healthy Athletes Fit Feet has not been able to hold in-person Fit Feet events with our Special Olympics Athletes---so as a means to stay in touch with the Athletes’ overall foot health, Fit Feet is utilizing Virtual Fit Feet where we can, using Zoom, chat with our Athletes about their concerns and if necessary direct them to an in-person provider for care. Pictured below are two Arizona College of Podiatric Medicine students, Morgan Shano and Cassie Wisbang as well as Special Olympics Arizona Intern Erika and Athlete Laralei.

Student Chapter Update

As everyone knows, this has been a difficult year for obtaining continuing education in person. AAPSM student chapter presidents, Kirk Metzger (Des Moines University) and Anthony Schwab (Temple University) approached the AAPSM about presenting a Zoom conference on sports medicine. We were able to arrange an evening meeting to discuss sports medicine and podiatry in general.  This is something the AAPSM board strives to do to enhance the sports medicine aspect of podiatric medicine. Although we do prefer to do our “Days of Sports Medicine” conferences in person, we were glad to connect with the students during this most difficult year.  Soon after these Zoom meetings, student chapter representatives from Western University, NYCPM and Scholl also reached out to host virtual seminars!

At DMU and Temple, the meetings encompassed starting a sports medicine practice and the make-up of a sports medical team. For the evening with Des Moines, current president Jim Ratcliff, DPM joined Howard Osterman, DPM and talked about running injuries in the older patient. Because of the format and mostly 1st and 2nd year students participating, the talk was geared towards general sports podiatry. We hope that future sessions will include more surgical and rehabilitation options. Podiatric sports medicine is one of the areas of medicine where our members really excel and are an integral part of any medical team. It is an excellent opportunity for our members to shine when working interactively with other sports medical professionals. The AAPSM always appreciates the opportunity to work with the students and look forward to advancing this in our current Young Member Mentor Program (YMMP).

So, special thanks to AAPSM Fellows Dr’s Howard Osterman, Jim Ratcliff, Alicia Canzanese and Jay Spector who have all generously donated their time to make sure that all of the podiatry colleges have the opportunity to host a virtual “Day of Sports Medicine”.  The students are always engaged, and we hope to get to every school and cover a variety of topics. For additional information or to coordinate a virtual “Day of Sports Medicine”, please feel free to contact Rita Yates at ritayates2@aol.com.

Journal Review

Delayed (14-30 Days) Percutaneous Repair of Achilles Tendon Ruptures Offers Equally Good Results As Compared With Acute Repair

Reviewed by Hayden Hoffler, DPM

This article evaluated post-operative outcomes of Achilles tendon ruptures fixed percutaneously either acutely (<14 days) or delayed (14-30 days). 42 patients were in the study with 21 in each group. All patients underwent the same technique with a Bunnel suture in the proximal stump and Kessler in the distal stump. Patients were immediately place in a below knee cast after surgery for 3 weeks followed by a functional brace that had 5 wedges with physical therapy. Over the course of a couple months, wedges were sequentially removed, and concentric and plyometric exercises were introduced. After 5 months at the patient’s discretion, return to activity was permitted. Patients were assessed at 6 weeks, 3 months, 6 months, and at a year. Tendon elongation, functional outcomes using the Achilles Tendon Rupture Score (ATRS), return to sports, calf circumference, muscle power, and complications were all measured.

Young Member Mentor Program

We Need Your Help!

YOUNG MEMBER MENTORSHIP PROGRAM (YMMP)

The AAPSM Young Member Mentorship Program (YMMP) is up and running and we need your help!  As we mentioned in previous newsletters, the YMMP is a voluntary program intended to foster a cooperative one-on-one relationship between a young/new podiatric professional interested in pursuing a sports medicine practice with an experienced AAPSM Member in good standing.

Mentees can be AAPSM Student Chapter Members currently enrolled in a college of podiatric medicine, resident members, or members in their first 5 years of practice.
The Mentor would be a current AAPSM member (with at least 5 years of professional experience and within 5 years of retirement) with a vested interest in sharing their sports medicine knowledge, and “paying it forward” as a role model to the young member.
